13-UT-006: Replace Existing Steel Plate of Manhole Concrete Cover

Scope of Work: Procure encroachment permit from DPW and hot work permit from the Navy Fire Department for the work on the manhole cover located at Marine Drive, Asan, Guam. Install traffic cones, traffic signage with lighted arrows to signify the re-routing of traffic on the area. The cones will stretch 150′ from the working area (recommended by DPW) both the entry and exit where traffic will be re-routed. Concrete barriers will be installed at the working area. Pull-out the existing 3/4” thk. steel plate cover including 8 ea spikes and dispose. This procedure include the use of a 3.0ton for lift

Clean the area before installation of the new steel plate cover. Lay cold mix asphalt above the area of the existing damage valve box concrete cover to support the new steel plate. Carefully level and compact the cold mix asphalt. Install 3/4″ thk. x 7′ (w) x 20′ (L) MS plate on the top of the existing concrete cover and new cold mix asphalt. This procedure includes the use of 3.0ton for lift. Anchor the steel plate to the existing concrete cover by using 30 each spikes 1/2″ø x 10″ long and full weld on top of the steel plate.Apply cold mix asphalt around the perimeter of the steel plate and fully tampered approximately 18″ wide and will be leveled to the existing asphalt.
