Scope of Work: Fabricate replacement concrete vault cover, 75” x 67”, 10” thick, with #4 rebar 10” o.c. both ways, 2 layers in box configuration, 2” minimum cover on all sides; with cast iron manhole. Obtain road encroachment permit from DPW. Saw cut asphalt around perimeter of existing manhole cover. Remove and dispose existing damaged concrete vault cover. Remove and dispose debris inside of vault. Install new concrete vault cover over vault, install and compact hot mix asphalt around perimeter of cover level with existing pavement.
